Do I Qualify for a Medical Marijuana Card in New Jersey?

Three questions determine whether you qualify for a medical cannabis card in New Jersey: Do you live in New Jersey? Do you have a condition on the state's approved list? Is that condition documented by a physician? If all three answers are yes, you qualify. The evaluation at PremierMD confirms the clinical picture and issues the certification.

The Three Requirements

1. You Must Be a New Jersey Resident

The NJ Medicinal Cannabis Program is open to New Jersey residents only. You must have a valid NJ address and present a New Jersey ID or driver's license at your evaluation. Patients living near state borders — in areas that straddle Pennsylvania, New York, or Delaware — are common at PremierMD. Residence in NJ is the determining factor, not proximity to a PremierMD location.

2. You Must Have a Qualifying Medical Condition

New Jersey approves 18 conditions for the program. The full list:

Chronic pain, anxiety, post-traumatic stress disorder (PTSD), cancer, migraine, amyotrophic lateral sclerosis (ALS), inflammatory bowel disease (including Crohn's disease), intractable skeletal muscular spasticity, multiple sclerosis, muscular dystrophy, opioid use disorder, HIV/AIDS, glaucoma, seizure disorder (including epilepsy), sickle cell anemia, terminal illness (prognosis fewer than 12 months), Tourette syndrome, and dysmenorrhea.

If your condition is on this list, you have a qualifying diagnosis. If it is not on this list, you do not qualify under current program rules, regardless of symptom severity. The NJ Cannabis Regulatory Commission periodically reviews and adds conditions — sickle cell anemia was added in January 2026.

3. Your Condition Must Be Documented

A physician certifying you for the NJ program must verify your diagnosis through your medical records. Records from any licensed physician documenting the qualifying diagnosis are accepted: a primary care chart note, a specialist letter, a hospital discharge summary. The records need to show a diagnosis, not just a report of symptoms.

You do not need to be an existing PremierMD patient — records from another provider count. Bring whatever documentation you have; your PremierMD provider reviews it at the evaluation.

Common Questions About Eligibility

I have chronic pain but no formal diagnosis. Do I qualify?

Chronic pain as a qualifying condition requires documentation of a chronic pain diagnosis, not just a symptom report. If a physician has documented chronic pain in your medical record, you have what you need. If you have been managing pain informally — without a physician visit that resulted in a written diagnosis — you will need to establish that record before your cannabis evaluation.

PremierMD is a full-service family medicine practice. If you do not have a chronic pain diagnosis on record, the practice can evaluate you for that as part of a primary care visit before or alongside your cannabis evaluation.

I have anxiety but have never seen a psychiatrist. Does that count?

Yes. Anxiety does not require a psychiatrist's diagnosis. A documented anxiety diagnosis from any licensed physician — including a primary care doctor — qualifies. If anxiety has been noted in your primary care chart, those records are sufficient.

My condition is well-managed. Do I still qualify?

Yes. Qualifying conditions do not need to be actively severe or untreated. A managed diagnosis of cancer, MS, or chronic pain still qualifies you. The evaluation assesses whether medical cannabis is clinically appropriate for your current situation — not whether your condition is at its worst.

I take opioids for pain. Does opioid use disorder qualify on its own?

Yes. New Jersey added opioid use disorder as a standalone qualifying condition to allow patients to use medical cannabis as an alternative or complement to opioid-based pain management. You do not need a second qualifying condition. A documented opioid use disorder diagnosis is sufficient.

I am a veteran with PTSD. What documentation do I need?

Your PTSD documentation from the VA — a VA rating letter, a mental health treatment summary, or a discharge document citing PTSD — serves as supporting records. VA documentation is accepted. Veterans at PremierMD routinely use VA records for their evaluation.

My condition is not on the list. Is there any other path?

Not currently. The NJ program operates from a fixed approved conditions list. If your condition is not on the list, you cannot be certified under current program rules. The CRC can add conditions through a formal review process — check the NJ Cannabis Regulatory Commission for updates.

What the Evaluation Actually Looks Like

Once you confirm you meet the three requirements, the evaluation at PremierMD is a standard physician visit — a clinical conversation, not an interrogation or a formality. Your provider reviews your records, discusses your condition and symptoms, explains how medical cannabis applies to your specific situation, and addresses questions about dosing, product selection, and what to discuss with your dispensary pharmacist.

Your PremierMD provider — Dr. Boguslavsky or one of the practice's board-certified NPs or PAs — certifies you the same day if you qualify.

Evaluations are available in-person at four NJ locations (Bridgewater, Morristown, Nutley, Hoboken) and via telehealth for patients anywhere in New Jersey. Medicare, Medicaid, and most commercial insurance plans are accepted. Most patients pay only their standard copay.

What Happens After You Qualify

Same-day certification: Your provider issues a written certification at the end of your appointment.

State registration: Register through the NJ Medicinal Cannabis Program patient portal. The digital patient card is free; a physical card costs $10.

Card processing: Most applications process within one to two weeks per the NJ Cannabis Regulatory Commission. Once your card is active, you can purchase from any licensed NJ dispensary.

Annual renewal: Your card must be renewed each year. The renewal visit follows the same process as the initial evaluation and is also billed to your insurance.

Frequently Asked Questions About NJ Cannabis Eligibility

Can I be denied a certification even if I have a qualifying condition?

Yes. The evaluation is a clinical assessment. If the provider determines that medical cannabis is not clinically appropriate for your situation — due to drug interactions, complicating factors, or clinical judgment — they can decline to certify. This is uncommon, and PremierMD providers explain their reasoning when it happens.

Is there an age requirement?

You must be 18 or older to be certified as a standard patient. Patients under 18 may qualify in limited circumstances with parental consent and additional requirements — contact PremierMD to discuss your specific situation.

Do I need a referral from my primary care doctor?

No referral is required. You can book directly with PremierMD.

How long does the whole process take from eligibility check to card in hand?

Typically one to two weeks. The evaluation can happen the same week you book; state registration takes one to two weeks after certification.

Can I be evaluated for multiple qualifying conditions at once?

Yes. If you have more than one qualifying condition, your provider evaluates the full clinical picture. Having multiple qualifying conditions does not complicate the process and can strengthen the clinical rationale for certification.
