The four steps
Check Your Eligibility in Minutes
Fill out our short intake form — it takes about five minutes. Tell us your primary condition, your insurance, and which location is most convenient for you.
Within one business day, a member of our team will reach out to confirm your insurance coverage and schedule your appointment. No commitment required.
Your Evaluation — Usually by Video, From Home
The vast majority of our medical cannabis evaluations are done by telemedicine. We'll send you a secure link — no app to download, no account to create. You join on any smartphone, tablet, or computer with a camera.
During your visit, your provider will review your medical history, discuss your symptoms and previous treatments, and determine whether medical cannabis therapy is clinically appropriate for your situation. If you prefer to come in person, all four of our NJ locations are available.
This is not a five-minute rubber stamp. This is a real medical appointment with a board-certified medical professional who takes the time to know you — just done from your living room.
What to have ready:
- Your NJ driver's license or state ID (hold it up to the camera)
- Your insurance card
- Your current medication list
- A quiet, private space with decent internet or cellular signal
- Any relevant medical records — helpful but not required
Get Your NJ Medical Cannabis ID
Once your provider certifies you, we submit the recommendation directly to the NJ Cannabis Regulatory Commission (CRC) through the official NJ Medicinal Cannabis Program (NJMCP) portal.
You will receive an email from PremierMD with instructions on how to register on the CRC portal, along with your reference number and registry ID number. Once you register with the CRC and submit your information, you will be issued a physical or virtual card (your choice). This ID allows you to purchase medicinal cannabis at any licensed alternative treatment center (dispensary) in New Jersey.
Timeline:
- CRC processing typically takes less than a week
- If you choose a digital ID card, it arrives almost instantly upon CRC approval
- Your patient card is valid for 2 years — renew directly on the CRC portal when it expires
- Physician recertification visits are required every 3 months — covered by insurance in most cases

We schedule quarterly follow-up appointments to assess how your treatment is working, adjust recommendations as needed, and address any concerns. Our team is available to answer questions between visits.
When your annual recertification comes due — required by NJ state law — we make the process seamless. It's typically handled at one of your regular quarterly visits, often covered by insurance.

No. We welcome new patients specifically for the medical cannabis program. If you are already a PremierMD patient for other care, your records are already in our system, which typically makes the process faster.
If your provider determines that cannabis therapy is not clinically appropriate for you, they will explain why and discuss alternative treatments. Because we are a full-service practice — not a certification-only service — we can continue to support your care through other means.
