Step 1 — Confirm your eligibility.

Step 2 — Register and book a telehealth appointment.
Register as a patient and select a telehealth slot. Telehealth appointments are available across the practice's schedule and cover patients anywhere in New Jersey.
Step 3 — Prepare for your visit.
Have your insurance card, a photo ID, and any medical records documenting your qualifying condition available before the call. Records from a prior treating physician for your qualifying condition — a primary care chart note, a specialist letter, a hospital discharge summary — support the evaluation and can speed it up.
Step 4 — Attend your video appointment.
Your PremierMD provider — Dr. Boguslavsky or one of the practice's board-certified NPs or PAs — conducts a full clinical evaluation via secure video. They review your history, discuss your symptoms, assess whether medical cannabis is appropriate, explain how cannabis applies to your specific condition, and answer your questions about use, dosing, and what to expect at the dispensary.
Step 5 — Receive your certification and register with the state.
If you qualify, your provider issues the certification at the end of the appointment. You then register through the NJ Medicinal Cannabis Program patient portal. Your patient card — digital (free) or physical ($10) — arrives within one to two weeks per the NJ Cannabis Regulatory Commission.
